Preschool Quick Check: Child must be 3 years old by August 15th to enroll in Cubbies and be MUST be fully toilet-trained. (Children who are 5 years old and not yet in Kindergarten will enroll in Cubbies since older Awana clubs are based on school grade. This can be discussed with the Awana Ministry Director if necessary.)

Participation Clause: My child is granted permission to attend and participate in the Grace Baptist Church Mount Juliet Awana Club. This includes any in-house and/or off site events and activities that are planned by its leadership, staff, and/or volunteers. Furthermore, I release and agree to hold harmless Grace Baptist Church Mount Juliet, its leadership, staff, volunteers, and Awana International, for any and all liabilities or claims for personal injury or illness; this includes loss of possessions that may occur when my child is present. (PLEASE NOTE: Every participant is encouraged to use good manners and to be responsible for their belongings. Please be sure when you pick up your child that he/she has all her belongings before leaving Awana.)

Sick Policy: Our policy is plain and simple; if your child is sick with any of the symptoms listed PLEASE do NOT bring them to Awana. Symptoms might include; fever, chills, coughing, nausea, vomiting, diarrhea, sore throat, runny nose, etc... We do not administer medication to Clubbers.

Medical Release: Emergency and non-emergency treatment may be given to your child when absolutely necessary. A reasonable effort will be made to contact Parent(s), Guardian(s), or Emergency Contact(s). FOR EXAMPLE: 1) A child complains of a headache or has a slight fever; parent/guardian/emergency contact will be called for assistance. 2) A serious medical emergency occurs with a child; emergency personnel will be called for assistance first, then the parent/guardian/emergency contact immediately thereafter or during the same time. NOTE: We do not administer medication to Clubbers.
